On Tuesday, at our special "Mind the Gap" conference, nurses, aides, doctors and other staffers discussed communication problems that plague the health care system. Gaps occur when patients go from the ICU to rehab or go home after surgery — all without the proper information.

Our goal is to help close patient information gaps with correct medical data and with compassionate care.
