If you would like to pick up your medical records personally and are not transferring them to another provider, please stop by the office to sign a release form. There is a $25 fee, and once the form is completed and payment is received, it may take up to 14 days for your records to be ready for pickup.

If you are in need of a medication refill, please contact your pharmacy and have them send us a refill request.

At this time, we do not have any appointments available, including for urgent concerns. If you are experiencing an emergency, please call 911 or go to your nearest emergency room.
