Access to compassionate, quality end-of-life care should not depend on someone’s zip code, race, background, language, or financial situation. Yet many communities still face barriers to education, support, and care.

At Hospice Foundation for Outreach, we believe advocacy, education, and honest conversations are key to creating a more equitable future for individuals and families navigating serious illness.

One important conversation to consider before move-in day? Advance healthcare directives.

Once your child turns 18, parents may not automatically be able to make medical decisions or access health information in an emergency. Completing these documents ahead of time can provide clarity, peace of mind, and help ensure their wishes are known.

It’s not always the easiest conversation, but it’s an important one. 💙
