Our friends at the Constellation Regional Collaborative are doing great work within the community.

❤️🩺 Agner Munoz-Vazquez Muñoz, Community Health Specialist with the Constellation Regional Collaborative (CRC), participated in the Feria de Salud hosted by Casa de Pan y Alabanzas in Jamestown, NC, alongside organizations dedicated to improving community health and well-being.

🤝Community members received education and hands-on practice using the Healthy Heart Ambassador Self-Measured Blood Pressure (SMBP) program, helping them build confidence in monitoring their blood pressure at home and better understand the importance of cardiovascular health management.

Through community outreach and partnerships like these, CRC continues to support health education, strengthen community-clinical connections, and empower individuals to take an active role in their heart health.

Constellation Quality Health is now accepting nominations for the 2026 Harris Memorial Award, recognizing outstanding physicians who demonstrate excellence in patient care, leadership, innovation, and service.

The selected recipient will be honored by their peers and receive a $5,000 charitable donation to the organization of their choice.

Did you know that high blood pressure affects people of all ages? About 1 in 3 Americans have it, but don’t even know it. The National Heart, Lung, and Blood Institute (NHLBI) encourages everyone to take action to prevent or control high blood pressure.

7 Ways to Keep Your Blood Pressure Healthy

You can start by adopting heart-healthy habits, including:

· Being more physically active
· Maintaining a healthy weight
· Following a heart-healthy diet such as the Dietary Approaches to Stop Hypertension (DASH) eating plan
· Quitting smoking
· Managing stress
· Getting 7-9 hours of quality sleep
· Knowing and tracking your heart health numbers, like blood pressure, cholesterol, and blood sugar levels

You can stay aware of your blood pressure by having it checked at each visit with your healthcare provider. A healthy blood pressure is typically less than 120/80 mm Hg.
