This ensures that we have all your information and medical history in advance, allowing our therapists to be better prepared for your session. Accessing these forms is as easy as a few clicks on your computer or phone!

✨ Unsure of what to wear? Opt for comfortable and breathable clothing! If you're visiting us for a knee-related concern, it's best to wear attire that allows the therapist to examine your knee. Our priority is your comfort, especially during any tests or exercises we may perform.
