Our Tribal Care Coordination team supports American Indian and Alaska Native (AI/AN) members enrolled in the Oregon Health Plan (OHP) with personalized care that respects Native cultures, community and individual needs.

Tribal Care Coordinators help make it easier to access health services, connect to community resources and navigate your benefits. They build trust and strong relationships, making sure that AI/AN members feel comfortable and confident about their health.
