If you have hit your 40s and your brain suddenly feels like it is working against you, you are not imagining it.

Focus that used to come easily starts to slip. You forget why you walked into the room. The overwhelm arrives faster, and small things feel louder. For a lot of women, this gets brushed off as stress or just getting older. The real story often traces back to what perimenopause does to the brain.

On Thursday, June 18 at noon ET, Marla Osner, CRNP sits down with Avery Wasmanski, LPC, a therapist who works with women with ADHD and neurodivergent women, for an honest conversation about perimenopause and the ADHD brain. What is actually happening, why it gets missed, and what you can do about it.

She went to her own doctor with brain fog, mood swings, and a gut feeling something was wrong.

She was told she was fine.

She's a nurse practitioner with decades of clinical experience — and she was still dismissed.

That moment changed everything. Marla Osner didn't just find her own answers. She opened a clinic. Because if this was happening to her, she knew it was happening to thousands of women who didn't have her background to fall back on.

In this week's episode of Beyond Hormones, Marla shares what that first year of clinic ownership looked like — the underpricing, not paying herself, learning that loving your mission doesn't automatically translate to business savvy. She also walks through the whole-body model she's building at Eclipse Total Health: pelvic floor PT, somatic coaches, nutritionists, personal trainers — a real integrated approach that goes far beyond just the hormones.

And then she says something that stopped me mid-conversation: "Resilience is a decision. It is not motivation."

That's the whole episode right there.

If you've been thinking about making the leap from provider to practice owner, or if you're already in the middle of building and wondering if it's supposed to feel this hard — this one's for you.

If you have ever started strong on a health plan and then stopped, this is for you.

It is not a willpower problem. When your nervous system is stuck in survival mode, sticking to healthy habits becomes genuinely hard, and no amount of trying harder fixes a dysregulated nervous system. That is not laziness. It is biology, and the good news is it can change.

Last week, I sat down with Ashley Basilio at Get Fit With Ashley to walk through what is actually happening in your body and the first steps to calming it. The full workshop is now in our library.

"I feel heard, seen, and supported in a way that is incredibly rare in modern medicine."

That's how Johanna described her care at Eclipse Total Health, and it's the part that matters most.

So many women come to us after being handed normal lab results while still feeling exhausted, foggy, and not like themselves. Being dismissed is its own kind of exhausting.

Marla takes the time to listen, explain, and truly understand what's going on. Perimenopause is not something you have to white-knuckle alone, and you deserve care that sees the whole picture.

I hear similar stories to this daily, but this was actually my story......

I want to tell you what not okay actually looked like before I tell you how I found my way out of it, because not okay doesn't always look the way people expect. It doesn't always look like falling apart. Sometimes it looks like a woman who gets up every morning and does everything that needs doing and then collapses the moment the doing stops.

Every Sunday was the same. I would wake at five or six in the morning after hours of tossing and turning on a mattress that felt like it was fighting me, my hips aching in the specific way of someone whose body is staging a quiet revolt. I would walk half crippled to the kitchen for coffee, holding the counter while it brewed, waiting for something to kick in. A few chores. Maybe a load of laundry. By ten I was back down for an hour or two, not restful sleep but the heavy unavoidable kind that takes you whether you want it or not. Then up again. A television show I couldn't follow. Another attempt at something productive. Then down again.

Sometimes the nap happened in the car, parked in a lot waiting for James to finish training, the hot sun coming through the windshield, too tired to move to the shade, just gone. A woman who ran emergency rooms and managed crises and held other people's worst moments together, asleep in a parking lot because her body had simply run out of caffeine and bad decisions.
Cabinet doors left open. Half packed lunches abandoned on the counter. Words that dissolved between my brain and my mouth before they finished forming. A fatigue that wasn't tiredness, it was something deeper and more total, like the pilot light had gone out and I was just running on residual heat.

The above is an excerpt from a book I am writing currently... sometimes we are not ok, and I was not. I wasn't taken seriously by my providers. So I set out to fix myself. This is how Eclipse was born. And I strive everyday to SEE and HEAR my patients so that they never feel the way I did.

Your body is doing something you didn’t sign up for.

Same eating. Same workouts. And somehow the scale is going up and your clothes fit differently. If that’s you right now, you are not imagining it, and you are not doing anything wrong.

Perimenopause changes how your body holds onto weight, especially around the middle. Shifting hormones rewrite the rules, and the things that used to work just stop working. That is not a willpower problem. It is a biology problem, and it deserves a real answer instead of another crash diet.

You don’t have to white-knuckle your way through this or figure it out alone. Understanding what’s actually happening is the first step toward feeling like yourself again.

If you’re ready for that, we’re here.
