This patient came in for a fertility follow-up, hadn't even missed her period. Melissa, one of our MAs, said go ahead and grab a urine for us. It's her first baby, several rounds into fertility treatments, and she burst into tears when she saw the positive pregnancy test!! This is one of our favorite parts of our job. In this episode of, "Why Can't I Get Pregnant: Follow the Infection," we have a couple with Secondary Infertility for 6 years. There are some hormonal things going on with both of them (her, PCOS and him, low testosterone).
-On history, she mentioned "odor," no other symptoms. He had no symptoms.
-His semen showed increased white blood cells. 🚩 Red flag number one, low s***m count. 🚩 Red flag number two, possible infection.
-Wife swabbed. She has Bacterial Vaginosis. We are now treating both partners for this.
-Husband semen sent for infection testing. He has Enterococcus and Group B streptococcus. He is treated as this infection is likely arising in the prostate or epididymis (connects the testicles to the vas deferens).
-They will rebuild microbiome with Liposomal Vitamin C & probiotics.
-Semen analysis will be repeated after infection cleared & testosterone normalized.
-Wife will likely need medicated blood ovulation cycles & luteal phase progesterone.
