Healthy Metric has just released a second statewide report this week! The Primary Care Utilization and the Health of Adult Women of Childbearing Age in Wisconsin report focuses on the health disparities among adult women of childbearing age (18-44) in Wisconsin. It provides critical insights into primary care visits, diabetes A1c control, blood pressure control, and depression screening for this demographic. The Transfer Call Toolkit is now available on ! It offers tools to help center nurses facilitate calls about transfers and ensure that the information exchange is clear, complete, consistent, and efficient. It also provides to facilitate transfer center nurses on the tools and to communicate to providers and leadership at referring and accepting hospitals that the tools will be used.
