WPHCA is currently accepting nominations of Wisconsin Community Health Center teams for our 2025 Performance Excellence Award! The Performance Excellence Award recognizes and celebrates Health Center teams and the efforts they have made over the past year towards achieving the quadruple aim: improving the health of patients, improving the patient experience, improving the cost effectiveness of care and/or improving the staff satisfaction. WPHCA recognizes that improvements can come in many forms and nominated teams may have contributed to a variety of achievements, including clinical and organizational process improvements, innovations in digital tools and technology, service expansion, and more. Please share widely and plan on nominating a Health Center team for their work! Nominations are accepted through August 1, 2025. Nominated teams and projects will be recognized in WPHCA publications leading up to WPHCA’s Fall Learning Session on September 17-18, 2025, where the award will be conferred.
