Hi everyone,
My 501c3, The Marigold Foundation, provides financial assistance to families in Southwest Wisconsin who have recently lost a pregnancy or infant, or who are seeking life-saving treatment for a pregnant person or their baby. The last time we collected data was in 2019, and it was limited to the Madison area. We're looking to update our data and would love it if you could take this survey and/or share it. This survey is meant for anyone who lived in SW Wisconsin at the time of their loss or who sought treatment here. This includes the following counties: Dane, Grant, Green, Iowa, Jefferson, Lafayette, Rock, Sauk, and Walworth. The survey only takes about 10 minutes and will help us continue to support our fundraising efforts to continue providing our services. Thank you!

The Department of Health Services, Division of Public Health, is hiring a Maternal Mortality Nurse Abstractor to develop and implement prevention recommendations based on mortality reviews.
