*Please provide a copy or attach the following documents with your application. No application will be accepted without these:

-Transcript from college or high school for the most recent academic year
-Physician statement confirming cancer and date of diagnosis
-Program acceptance letter or enrollment verification
-100-word essay answering the following question: Why should you be selected to receive this scholarship?
