Update on Nookie's left ALD correction. There were lessens learned from the right side. With such severe torsion it is hard to gauge soft tissue constraints and laxity with a single acute correction. For the right side we had a cutting guide and then a reduction guide that was the "finished" correction and we had to de-rotate the bone to apply the reduction guide. The was a very difficult task.

So for the left side we designed a reduction guide that had 10 degree increments so we could slowly rotate, allow the tissues to stretch then relax and evaluate, Once the alignment was accurate we could also test for carpal laxity. The new guide design allowed us to correct the deformity much quicker in the operating room and made the surgery itself (while challenging), easier.
