Today our residents took part in a water balloon fight, and for a little while they got to put aside the weight of everyday challenges, laugh together, and simply enjoy the moment. Recovery and healing happen in many different ways, and positive social interaction is an important part of that process.

At Community Missions, we believe peer support is about creating opportunities for connection, reducing stress, and reminding people that joy still has a place in their journey. Sometimes, a little laughter really is the best medicine. 💙💦

**Hours:** Monday–Friday, 10:00 AM – 6:00 PM (1-hour unpaid lunch)

# # # About Community Missions

Community Missions Corporation operates the area's only 24-hour low-barrier emergency shelter for men and women, along with permanent supportive housing, street outreach, and supportive services designed to help individuals experiencing homelessness achieve stability and self-sufficiency.

# # # Position Summary

Community Missions is seeking a compassionate, motivated, and organized individual to serve as a **Case Manager / Outreach Specialist**. This position works directly with individuals experiencing homelessness by providing case management services, conducting outreach throughout the community, connecting clients to resources, and assisting individuals in achieving housing and personal goals.

# # # Essential Duties and Responsibilities

* Conduct assessments and develop individualized service plans.
* Meet regularly with shelter residents and outreach participants to monitor progress and provide support.
* Assist clients with obtaining identification documents, benefits, employment, healthcare, treatment services, and housing resources.
* Conduct outreach to individuals experiencing homelessness in camps, public spaces, hospitals, and other community locations.
* Complete Coordinated Entry assessments and referrals as needed.
* Maintain accurate and timely client documentation in HMIS and agency databases.
* Collaborate with community partners, landlords, treatment providers, healthcare organizations, and government agencies.
* Facilitate life skills, housing readiness, and supportive service groups as assigned.
* Advocate for clients while promoting self-sufficiency and personal responsibility.
* Participate in community meetings, case conferencing, and agency trainings.
* Assist with transportation of clients when necessary.

# # # Qualifications

* Associate's degree preferred; experience in social services, human services, criminal justice, public health, or related field considered.
* Experience working with vulnerable populations preferred.
* Strong communication, documentation, and organizational skills.
* Ability to work independently and as part of a team.
* Knowledge of community resources and homeless services is beneficial.
* Valid driver's license and acceptable driving record required.
* Ability to pass background screening.
