This patient came in to “Evergreen Endodontics” for an emergency visit with real tooth pain, and it wasn’t random. Two teeth had a history of deep decay that had been treated with fillings, but the original cavities were close to the nerve, and over time both nerves became inflamed. His dentist saw long, sharply curved roots on the X ray and referred him to Dr. Nima and our endodontic team here in Issaquah.
Before we started the root canal, we used 3D imaging (CBCT) to map the roots. It revealed an extra hidden canal that can be missed on a standard X ray, and that can lead to lingering symptoms later.

Check out this challenging case! This patient came in with a crown on tooth number 30, and one of the canals (the Mesio Buccal) was calcified due to the canal being too close to the crown margin. A 3D CBCT scan and a microscope were essential to locate it. The tooth was also exceptionally long... over an inch (25 & 26 mm roots) from crown to root tip, placing it among the longest we’ve seen for a molar. We successfully completed the root canal, relieved the patient’s pain, and wanted to show you the kinds of complex cases we handle. A patient came in with a painful upper back molar (tooth #2), and this one was definitely unique! The root was not only super long - much longer than usual - but it also had a sharp “dog-leg” curve.
