Responsibilities

The RN / LPN MDS Coordinator is responsible for completing minimum data set assessments, Care Area Assessments and collaborating with the resident to develop comprehensive plans of care after review of the medical record and communication with resident, direct care staff, the physician and Resident Representative. The RN / LPN MDS Coordinator facilitates the assessment process in accordance with the requirements of federal and state regulations and company policies and procedures.

Skills and Abilities:

Strong clinical assessment skills
Working knowledge of Medicare and Medicaid reimbursement
Effective interpersonal skills and the ability to work with an interdisciplinary team
Requirements are:

LPN or RN license
Prior experience as an MDS CoordinatorO

RN (Registered Nurse) ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:

Assesses patients by physical examination including pertinent diagnostic testing to determine health status.
Administers medications and treatments.
Participates in the care planning process and oversees implementation of the plan.
Supervises LPNs and Nursing Assistants. Oversees ADLs and documentation.
Communicates with physicians regarding changes in conditions, diagnostic test results, etc.
Documents assessments and care in compliance with standards of care and company policy.
Educates patients and their families on health-related issues.
Graduate of an accredited school of Nursing.
Valid State licensure as an RN.
Current CPR certification.
