Bridges Program - Frederick Health

Frederick Health's Bridges Program provides information on the prevention of chronic disease and illness to representatives of the minority, underserved, disability, stigmatized, senior and most vulnerable communities such as those who cannot speak or read English. The recent Pandemic has highlighted and increased the gap of health disparities in these communities.

Communication is our greatest weapon in fighting this disease so that everyone knows how to stay protected from the disease and avoid spread. We see that clear, factual information may not be reaching some because of language or communication barriers, isolation from regular friends, community interactions and informational events, lack of access to free screenings, or problems finding a primary care provider.
