Most metabolic advice is written for someone else’s biology. The bodybuilder eating 400g of carbs. The athlete who never gains weight. The person who “can eat anything.” They are not the template. They are the exception. And the exception exists because of infrastructure — muscle mass, daily glycogen depletion, a liver that hasn’t been chronically overloaded. Most of my patients don’t have that infrastructure. They built their diet around people who do. That’s not a willpower failure. That’s a mismatch between the advice and the biology receiving it. Many of my patients come to me with severe insulin resistance already entrenched. Their muscle is no longer an effective glucose sink. Their liver is already overwhelmed. For them, carb restriction isn’t a preference or a trend — it’s the only lever that stops the damage and starts the reversal.
