🚧 What’s Up Wednesday: Are All Accidents Avoidable? 🤔
It’s one of the most debated statements in safety:
“All accidents are avoidable.”
If 99% was good enough in aviation or healthcare, the results would be unacceptable. “Almost safe” is not safe enough when lives are on the line.
But instead of debating extreme scenarios, let’s focus on what we can control:
✅ Planning the work
✅ Identifying hazards early
✅ Using leading indicators
✅ Reviewing processes and workflows
✅ Addressing unsafe behaviors
Incidents often occur when we fail to plan. When we apply a Plan, Do, Check, Act mindset and focus on eliminating unsafe behaviors, we dramatically reduce risk.
Maybe the better goal isn’t just zero injuries.
Maybe it’s zero unsafe behaviors.
