COMMUNITY MEMBER STORY

One morning, a Vista Springs community member who was normally active and engaged did not participate in his usual social programs. While this alone was not cause for alarm, it represented a noticeable change from his normal routine. Because participation and engagement are documented by our Lifestyle Experience Partners, the change was quickly visible within the Vista Springs care ecosystem. Team members communicated internally, a wellness check was completed, and the community member was found to be experiencing unusual fatigue and discomfort.

The Health & Wellness team was immediately notified and an assessment was completed. Based on their findings, the community member's physician was contacted, and the Vista Springs Rapid Response Team coordinated the next steps to ensure timely medical evaluation. The community member was transported to the hospital, where physicians determined he was experiencing a cardiac-related event that required prompt treatment. Because the change was recognized early, care was coordinated quickly, family members were notified, and the appropriate medical intervention was provided before the condition became more serious.

Through the Family Engagement App, loved ones received timely updates and were able to communicate directly with the Vista Springs team. Family members met him at the hospital, confident that the right steps had already been taken and that communication was occurring every step of the way. Following treatment and stabilization, the community member returned home to Vista Springs, where his care and service plan were updated and additional wellness supports were put into place.

His daughter later shared:
"What impressed me most was how quickly everyone came together. Before we even arrived at the hospital, Vista Springs had already assessed the situation, communicated with us, coordinated care, and made sure my father received the attention he needed. We never felt alone."
