All adults are now eligible for the COVID-19 vaccine.

PMG offers all three vaccines (Pfizer, Moderna, J and J) as supplies become available.

All COVID-19 vaccines are free! You will NOT receive a bill! You do NOT have to establish care with one of our providers to receive a shot!

PMG now has COVID-19 vaccine in stock. If you are 70 years of age or older you are eligible to be vaccinated.

If you are over age 70 please TEXT us at 970-372-0456 to be scheduled.

You will be given 2 appointments, one for the first dose and a second appointment three weeks later for the booster shot.

We CANNOT vaccinate ANYONE under 70 at this time. We recommend you sign up with San Juan Basin Public Health to be notified when you are eligible for a vaccine.
