The past two years has been one of the most challenging periods ever for HQP. We were hit by a pandemic that viscously targeted our primary service population - vulnerable, older adults with complex needs and chronic conditions. Our nurses never wavered and never stopped seeing patients who needed us at home or in the nursing home - even at the start of the pandemic when most other health care providers drastically limited their service. Why? Because as a unique organization that not only provides care, but is dedicated to learning what really works to prevent avoidable complications, disability, and death ... we knew we were significantly contributing to a better health outcome during a time of great need. We follow the data.

There have been many other hardships HQP has endured during this time, including major losses of funding from organizational partners who suffered financially due to COVID. But, so far, with the help of old friends, new guardian angels and emerging partnerships we press on. Relentless in pursuing our aim to design, test, and spread advanced preventive care systems that help keep us all well ... no matter the challenges.
