Is the PSA test really the best way to screen for prostate cancer? Not by itself. A high PSA is suspicious, but it can be raised by many ordinary things, including an enlarged prostate, an infection, even a recent bike ride, so on its own it can't tell you whether a biopsy is truly needed. The screening pathway is evolving to answer that question more precisely.

A new study in BJU International finds that active surveillance significantly outperforms nerve-sparing prostatectomy when it comes to preserving both sexual and urinary function, reinforcing that for the right patient, waiting and watching remains a very valid choice.
