As of May 6, 2023 I will be transitioning to an Associate Optometrist with Walmart! So I will be out of network with all vision and insurance plans temporarily on a case by case as Walmart brings me in network with the plans they accept. Please contact the clinic @ 801-779-3805 for more information as it's updated. I thank all my patients for their patience during this transition.
