Illinois is making a huge impact in the healthcare industry with the Home Centered Care Institute’s (HCCI) Illinois House Call Project (ILHCP). 14 medical practices have worked hard to provide care to nearly 200,000 residents who need home-based primary care (HBPC). In just one year, these Champions have already served 1,732 new patients, putting them over halfway to their three-year goal of 3,000!
