Thresholds Health, a division of Thresholds providing primary care services, has officially transitioned operations to PCC Community Wellness Center. PCC is a trusted Federally Qualified Health Center with a strong track record of delivering affordable, community-based healthcare across Chicagoland.

This transition allows the clinic to continue serving patients at its longtime West Side location, 5801 W Corcoran Pl, while also creating opportunities to expand services with PCC’s expertise and resources. For current patients, care continues with minimal disruptions or changes to care plans.
