Comprehensive Case Management: Working hand-in-hand with residents in supportive and affordable housing to navigate daily challenges and maintain long-term housing stability.
Connecting to Benefits & Entitlements: Helping tenants unlock critical resources by coordinating seamless referrals to internal FAC programs, affiliate staff, and trusted external partners.
Personalized Service Plans: Conducting needs-based assessments to collaborate with tenants, setting personal goals, and building a roadmap for their success.

HRA Benefits: Get help applying for SNAP (Food Stamps), Cash Assistance, Fair Fares (discounted metro cards), One-Shot Deals (emergency assistance), and more.
Housing & Utility Assistance: Access critical non-HRA programs like SCRIE/DRIE (rent freeze programs for seniors and people with disabilities) and HEAP (heating and cooling energy assistance).
Health Assessments: Evalevi conducts personalized health assessments to identify specific health-related resources and services tailored to your needs.

Walk-In Tuesdays: Just stop by and see us on Tuesdays from 9:00 AM – 3:00 PM.
