That’s why the SPARX Care Management Program exists.
We work with NYC families who have Medicaid and children from birth to age 21, helping them access ongoing medical and behavioral health care without navigating the system alone.

Through strong partnerships with doctors, clinics, and specialists, we help families get priority appointments and the services their children need sooner.
You’ll have a dedicated care manager who coordinates appointments, connects you with resources, and ensures your child’s care stays on track—every step of the way. ✅
