I did a really fun case today with BodyCad, utilizing patient specific instrumentation, guides, and custom plate designed precisely for my patient to perform a lateral opening wedge distal femoral osteotomy along with osteochondral allografts to the trochlea and lateral femoral condyle. I have always felt that we spend so much time planning these osteotomies only to then judge our correction with a bovie cord or long rod and multiple C-arm shots, having to deal with parallax, the rod or cord moving, and the overall lack of precision. Recent hip arthroscopy case on a young man with cam impingement. Around the time of adolescence a stress response at the growth plate of the proximal femur leads to excessive bone formation which grinds on the cartilage and tears the labrum which is the gasket around the hip joint. During hip arthroscopy I repair the labrum, treat the cartilage injury, and remove the impingement to stop the injury to the hip. This is a hip preservation procedure in hopes of staving off a hip replacement someday. In this particular case I harvested some of his own cartilage from the displaced flap of cartilage in his hip joint and grafted it back into the defect with cartilage matrix to promote cartilage regrowth.
