Do you need help managing your diabetes?

Health Extension for Diabetes is for people who have been diagnosed with either type 1 or type 2 diabetes.

The program provides participants with:
8 educational sessions
Self-management resources
Support to better manage diabetes

Regularly scheduled group meetings allow members to learn about meal planning, enjoyable activity, and blood sugar monitoring that help manage diabetes.

We have 2 upcoming cohorts to accommodate diverse schedules
