As of 01/01/2025, there is a form called Business Ownership Information Form. You must have this form completed and submitted. Any business that was structured before 01/01/2024 has until 01/01/2025 to complete this form. Any business form 01/01/2024 has 90 days to complete this form. There is a fine of $500 a day. This form is critical even if you are filing for business loss
