General Summary: This position is responsible for primary developing and maintaining good relationships, identify new areas of opportunity and act as a liaison for referral sources.

This individual promotes a harmonious relationship with Support Planners, Case Managers, Hospital Personnel, Physicians, Office Staff and Clients to ensure quality care.

Job Summary:

As an intake coordinator you will work in a team being responsible for:
*Communication and coordination with referral sources.
*Processing referrals with client's demographics, preferences and needs.
*Verifying client insurance eligibility.
*Scheduling field staff to deliver homecare services to clients.
*Work in partnership with care coordinator supervisor to ensure quality home care and excellent customer services.
*Communicate harmoniously with support planners on client care services.
*Making necessary changes and updates to supervising Care Coordinator.
*Complies with all agency standards, policies, and procedures in accordance with state and federal laws.
*Performs any other duties and assignments deemed necessary by the agency to ensure superior standards and quality health care.
