Burnt out PTs, this one’s for you.

I drove my 3rd grader to her field trip this morning. On a Wednesday . Not because I had a slow day. Because I built a cash-pay PT practice that actually works around my life.

I see fewer patients. I spend more time with each one. And I make more than I ever did grinding through a clinic schedule.

I was doing everything “right.”
Grad school. License. Clinic job.

And I was miserable.
Burnt out by Wednesday.
Counting down to the weekend.
Watching my PTO like it was a savings account I couldn’t touch.

So I burned the playbook and built my own with Concierge Orthopedics.

we need to talk about your foam roller.

if you’ve been grinding that thing up and down your outer thigh trying to “release” your ITB. I love the effort, but we gotta pivot.

here’s the thing: your ITB is not a muscle. it’s dense connective tissue, and research shows that foam rolling doesn’t actually change ITB stiffness at all. like, measurably zero change on ultrasound imaging.

why? because the ITB has no contractile fibers. it can’t “tighten” on its own and it can’t be “released.” what actually makes your ITB feel tight is the tension being pulled into it from the surrounding muscles and those are the ones that need the work.

rolling directly on the ITB also risks irritating the nerves and soft tissue underneath, so all that pain you’re pushing through? not productive. just pain.

roll here instead…
→ lateral quad (rotate slightly forward onto your front thigh)
→ lateral hamstring (rotate slightly backward onto the back side of your thigh)

foam rolling can absolutely be part of your routine. we’re not anti-roller. we’re pro-doing it right.
