Simple monitoring strategy to help recognizing readiness for weaning during peripheral VA-ECMO??
🔍 28 consecutive adults in VA , without intracardiac shunts or significant valvular regurgitation
🖥️ bilateral radial A-lines: pressure concordance (left–right systolic pressure difference < 10 mmHg, small MAP/DBP differences, similar waveform amplitude/morphology) + concordant ABG values suggested similar arterial blood source
🖥️ strong association with LVOT VTI meeting threshold commonly used for weaning assessment at subsequent
⚖️ bilateral pressure-gas concordance showed sensitivity of 94% + specificity of 50%, positive predictive value of 77%, negative predictive value of 83%, overall accuracy of 78.6%
Emergence of left radial pulsatility + bilateral radial pressure-gas concordance may serve as low-invasive, real-time bedside marker to facilitate early recognition of cardiac recovery, prompting formal echocardiographic assessment (rather than stand-alone marker of weaning readiness).

Distal limb perfusion in peripheral VA and minimally invasive cardiac surgery with femoral arterial cannulation??
🔬 in vitro study aimed to evaluate flow performance/pressure loss of introducer sheaths with side ports (widely used) and catheter w/o side port
🩸 flow augmentation limited primarily by resistance at side port region; catheters w/o side ports exhibit more linear pressure-flow relationship without saturation
🩸 catheters w/o side ports may represent potential alternative when conventional devices provide insufficient limb perfusion
🩸 side port structure is the dominant resistance point which accounted for the majority of total pressure loss: likely dominant role in limiting achievable antegrade flow. This may explain why DPCs may fail to resolve limb ischemia.
