This Facility is under strict guidelines and as required by The State of Alaska under mandate 16

Client pre appointment Covid 19 questionnaire as required by Alaska Mandate 16

1. Have you or any person living in your home traveled instate or out of state in the past 14 days?
2. Have you or any member of your household tested positive for COVID-19 in the past 14 days?
3. Have you or any person in your household displayed any COVID-19 symptoms in the past 14 days?
4. Have you had an elevated fever in the past 72 hrs?
5. Have you come in contact with any known person to have symptoms or that tested positive with COVID-19 in the past 14 days?

If you have answered yes to any of these questions by mandate we cannot preform services until all can be answered no.

1. Upon entry and exit please wash your hands for a minimum of 20 seconds
2. A face mask must be worn at all times prior to entry & during the duration of your appointment.
3. If possible please pay with a credit card. Exact amount of Cash will be accepted.
