Could be there more signs of "stickler"?
A 7yo child was referred to our Retinal department after a sudden deterioration of his right eye vision. A brief external examintion has showed that he had distinctive facial features (a scooped-out facial appearance and a receding chin, a cleft palate). He also had neurosensory auditory impairment, and a long eye axial (high myopic).
VA: OD=Cf., OS=NLP.
Left eye showed a dense cataract with posterior synechia and a long lasting funnel shaped RD on B-scan (image 3-4).
On his right eye, the fundus examination was not clear. But a superior temporal detachment was distinguished. He was scheduled for surgery to try to save his right vision.

راجعنا طفل 7 سنوات في قسم الشبكية بعد تدهور مفاجئ في رؤية عينه اليمنى. كان لديه ملامح وجه مميزة (ذقن صغير متراجع, وحنك مشقوق). كما كان لديه أيضًا نقص سمع حسي عصبي، وحسر عالي.
VA: OD = Cf. ، OS = NLP.
في العين اليسرى ساد كثيف مع التصاقات عدسية قزحية وانفصال شبكية طويل الأمد على B-scan (الصورة 3-4).
في عينه اليمنى ، لم يكن فحص القعر واضحا. لكن تم رؤية انفصال علوي صدغي. تقرر إجراء عمل جراحي لمحاولة إنقاذ العين اليمنى.
متلازمة ستيكلر هي السبب الرئيسي لانفصال الشبكية الشقي ثنائي الجانب عند الأطفال.

Diabetic papillopathy or papilloedema?.
A 39 years old female patient with type 1 diabetes reported to the clinic complaining of mild visual discomfort. Her BCVA OD:9/10, OS:7/10. Fundus examination revealed mild diabetic retinopathy + bilateral swollen discs. Diabetic papillopathy is uncommon, usually bilateral & occurs mainly in younger diabetics. It manifests with mild painless visual impairment. Bilateral disc swelling MANDATES the exclusion of raised intracranial pressure. The term papilloedema always refers to the swelling of optic nerve head secondary to raised ICP, which is not our case.
مريضة عمرها 39 سنة مصابة بالسكري نمط اول راجعت العيادة العامة بشكوى تدني خفيف في الرؤية. القدرة البصرية المصححة 9/10بالعين اليمنى و7/10 باليسرى. فحص القعر يبدي اعتلال شبكية سكري + توذم عصب ثنائي الجانب. اعتلال حليمة العصب السكري غير شائع، يكون عادة ثنائي الجانب ويحدث لدى السكريين الأصغر سناً. يتظاهر بتدني خفيف وغير مؤلم بالقدرة البصرية. وذمة العصب ثنائية الجانب تستوجب دوماً نفي ارتفاع التوتر داخل القحف.
