* Colored xerox of PSA Birth Certificate
* Colored xerox of PSA Marriage Certificate (if married)
* Xerox of any valid ID
* Three (3) 1x1 picture
* Four (4) passport size ID picture with white background, with name tag
*Must have an APPROVED BSRS ACCOUNT (please watch this video on how to make your own bsrs acct)
