• Photocopy of Birth Certificate
• Registration Fee

IMPORTANT REMINDERS:

1. Please note that only those who have completely filled out the enrollment forms will be considered officially enrolled.
2. We gladly accept transferees and new students.
3. Enrollment is strictly on a first-come, first-served basis, so we encourage everyone to enroll early.
4. All enrollment transactions must be done in person at DHCLC school only.
5. We do not allow enrollment through chat, online messages, or any unofficial means without accomplishing the school’s official form.
