**Please bring photocopy/printed copies of the requirements listed below**

REQUIREMENTS FOR CERTIFICATION
1. Hemodialysis Training Certificate from RENAP Accredited Training Center
2. Valid PRC License
3. RENAP Membership Card
4. Certificate of Employment
(At least 6 months clinical exposure or at least 3 months dialysis exposure)
5. PNA Membership Card
6. Proof of payment / Bank transfer
7. Seminar/Update Attendance (on-site)
8. 12 CPD Units for RETAKERS

Please bring any device for the exam
(Full charged Laptops/tablets/cellphones with sufficient mobile data, although wifi will be provided, bringing of powerbanks and extension wires are encouraged, NON-APPLE products are preferred to be used)
