All qualified examinees are advised to submit the following:

1. Original SF 9 (Report Card) or SF 10 (Student Permanent Records/form 137-A)
2. Original PSA Birth Certificate

Submit the following requirements in the Office of Admission and Registration in San Isidro Campus.

Submission of requirements for enrolment will start on June 16 - 30, 2025.
