• Original copy of Certificate of Good Moral Character from previous school
• Original copy and two (2) photocopies of Report Card (Form 138)
• Five (5) copies of 2x2 ID picture with white background

𝗙𝗢𝗥 𝗧𝗥𝗔𝗡𝗦𝗙𝗘𝗥𝗘𝗘𝗦 (𝗜𝗳 𝗘𝗦𝗖 𝗚𝗿𝗮𝗻𝘁𝗲𝗲):
• QVR Certificate & request letter of transfer credentials from the Records Office of the previous school.

Important Note: Students must be officially enrolled first before requesting a letter to claim their credentials from their previous school.
