2. Learnerโs Profile (SPS Form 3.2) with a 2x2 picture (See attachment at the comment section.)
3. PSA Birth Certificate or from the Local Civil Registrar
4. SF9 or Learnerโs Progress Report Card
โข Grades of 87 and above in Math, English, and Science
โข No grade below 85 in other subjects (From Grade 6, 1st to 3rd Quarter)
5. Certificate of Good Moral Character
6. Certificates of Academic and Non-Academic Awards (Arrange from the latest)
7. Declaration of Medical Condition Form (SPS Form 3.0) (See attachment at the comment section.)

March 16, 2026 - Qualified applicants will take the Mental Ability Test (30%) and the Communication Skills Test (30%).
March 17, 2026- Applicants will participate in Project Pitching (40%).
