𝗞𝗘𝗬 𝗗𝗔𝗧𝗘𝗦 𝗧𝗢 𝗥𝗘𝗠𝗘𝗠𝗕𝗘𝗥: 📅
- 𝗔𝗽𝗿𝗶𝗹 𝟮𝟬 – 𝗠𝗮𝘆 𝟮𝟵, 𝟮𝟬𝟮𝟲: Submission of initial required documents.
- 𝗝𝘂𝗻𝗲 𝟭, 𝟮𝟬𝟮𝟲: Official start of submission for 𝟮𝗻𝗱 𝗦𝗲𝗺𝗲𝘀𝘁𝗲𝗿 𝗔.𝗬. 𝟮𝟬𝟮𝟱–𝟮𝟬𝟮𝟲 grades.

𝗠𝗜𝗡𝗜𝗠𝗨𝗠 𝗤𝗨𝗔𝗟𝗜𝗙𝗜𝗖𝗔𝗧𝗜𝗢𝗡𝗦: 📋 Must 𝗡𝗢𝗧 have failing grades in 𝗜𝗻𝘁𝗲𝗿𝗺𝗲𝗱𝗶𝗮𝘁𝗲 𝗣𝗿𝗼𝗴𝗿𝗮𝗺𝗺𝗶𝗻𝗴, 𝗙𝘂𝗻𝗱𝗮𝗺𝗲𝗻𝘁𝗮𝗹𝘀 𝗼𝗳 𝗣𝗿𝗼𝗴𝗿𝗮𝗺𝗺𝗶𝗻𝗴, or other similar CS-related subjects.

Please ensure you have prepared all of the following documents before visiting the office:

1. Formal Letter of Request for Shifting
2. Official Report of Grades from the CRS (Note: First Semester grades are required for initial submission; Second Semester grades shall be submitted upon availability)
3. Photocopy of Student Identification Card
4. Photocopy of Parent or Guardian's Identification Card
5. Official Shifting Form (Available at the CISTM Office)
6. CISTM Curriculum Checklist (Available at the CISTM Office)
7. Proposed Study Plan with Student's Signature (Please consult the Department Chairman)
8. Note of Undertaking, if applicable (Available at the CISTM Office)
