Greetings!

Please be informed of the following deadline for the submission of the Co/Extracurricular Activities Rating forms for the Second Semester SY2025-2026. This is to give the SAO ample time to review and compute the rating forms submitted by the students.

For Graduating students - April 10, 2026 (Friday)
For Non-graduating students - May 11, 2026 (Monday)

In line with this, attached are the procedures to facilitate the preparation, submission and computation of the rating forms. Only forms properly accomplished and with supporting documents shall be considered for computation.

1. All Rating Forms shall be submitted by the Student in a Long Brown Envelope.
2. Envelopes of each student should be labeled as follows:
3. Only activities/events approved by the Student Affairs Office shall be accepted.
4. All submitted rating forms must be properly filled out by the students and signed by the activity adviser and/or a duty authorized person. Incomplete details will not be accepted.
5. Any of the supporting documents may be accepted by the Student Affairs Office:

a. Certificates with name (Participation, Appreciation, or recognition, etc.)
b. Certification from the activity adviser or faculty in charge (For approval of SAO)
c. Program and Invitation (if the student is part of the working committee or program)

6. School advisers shall submit a cover letter for the summary report signed by the School Dean/Department Head

As indicated in the official school calendar of CEU for the SY 2025-2026, February 17 (Tuesday) is declared a holiday in observance of Chinese New Year. Therefore, no classes shall be conducted on this day.
