SCHEDULE OF ENROLLMENT FOR THE SCHOOL YEAR 2024-2025

Admission Requirements for New Applicants (Freshmen)
o Filled Out Student Profile Sheet (From CSU)
(Original + 1 Photocopy)
o Advising form (given by the College dean after the interview)
o Admission Test Result Certification (Original + 1 Photocopy)
o Two (2) Long Brown Envelopes
o Four (4) Copies of Formal 2X2 ID Picture (Taken Within the Last 3 Months)
o NSO/PSA Birth Certificate (2 Photocopies)
o Photocopy of Marriage Certificate for Married Students (2 Copies)
o Form 138-A/Senior High School Report Card (Original + 2 Photocopies)
o Good Moral Certificate (Original + 2 Photocopies)
o Barangay Clearance

And, Any of the following:
o Certification from MSWD as an active member of 4P's or 4P's ID
o Certificate of Tax Exemption (if parents are Employed)
o Affidavit of No Fixed Income (if parents are Unemployed)
o Certification from OWWA (if parent or guardian is an OFW)

Additional Admission Requirements for Transferees
o Honorable Dismissal (Original + 1 Photocopy)
o Certificate of Grades/Transcript of Records
(Original + 1 Photocopy)

ATTENTION! TO ALL FUTURE CSUANS OF LASAM CAMPUS:

THE COLLEGE ADMISSION TEST WILL BE ADMINISTERED ON JUNE 22, 2024.

FOR ENLISTMENT, PLEASE COME IN PERSON TO THE CAMPUS ADMISSION OFFICE ON JUNE 18 TO 21, 2024.

PLEASE BRING THE FOLLOWING:

• Certificate of Enrollment
• Barangay certificate with 1x1 id picture
