✅ Step 1: Get the Admission Form and fill out the Student Cumulative Record – GUIDANCE OFFICE/ TESTING ROOM (beside Clinic)
✅ Step 2: Issuance of Student ID Number and Corporate Email – OFFICE OF THE STUDENT AFFAIRS AND SERVICES (OSAS)
✅ Step 3: Medical and Dental Examination – CLINIC
✅ Step 4: Submission of Requirements, Registration & Enrollment, and Release of Assessment Form – REGISTRAR'S OFFICE
✅ Step 5: Issuance of School ID – RESOURCE GENERATION MANAGEMENT OFFICE (RGMO)

📌 Enrollment Requirements:
• Original Copy of Grade 12 Report Card (Form 138);
• Original Copy of Certificate of Good Moral Character with seal;
• Photocopy of Senior High School Diploma;
• Photocopy of PSA/NSO Birth Certificate
• Three (3) Copies of 2x2 ID Picture (white background with nametag); and
• Long Brown Envelope
