Requirements for Admission:

1. 2 passport size picture
2. Long size brown brown envelope
3. Long size plastic envelope
4. Form 138 or Report Card
5. Form 137
6. Certificate of Good Moral Character
7. Photocopy of PSA Birth Certificate (please present original for comparison)
