Deadline for accreditation: OCT 23, 2023

🔷 REQUIREMENTS:
🔹OR CR (clear copy)
🔹DTI registration certificate
🔹BIR registration (BIR Form 2303)
🔹Copy of Official Receipt duly registered with BIR
🔹2 valid gov’t issued id’s (owner, driver, helper)
🔹Valid driver's license (driver)
🔹2x2 picture w/ white background (owner, driver, helper)
🔹Brgy. Health clearance or copy of Vaccination card (owner, driver, helper)
🔹NBI clearance / Brgy. clearance (owner, driver, helper)
🔹Medical exam with drugtest results (driver, helper)
