📢 NEW ONLINE PHARMACY SERVICES NOW AVAILABLE! 💊
The Quezon Provincial Health Network – QPHN Doña Marta (Atimonan) is now making healthcare more convenient through our Online Pharmacy Services, starting July 1, 2026.
No more long waiting times! Patients can now submit their prescription online and receive confirmation once their medicines are ready for pickup.
📝 HOW TO AVAIL OF THE ONLINE PHARMACY SERVICE:
Step 1: Scan the QR Code below or access the online request form.
Step 2: Fill out the required patient information completely and accurately.
Step 3: Upload a clear photo or scanned copy of your valid prescription.
Step 4: Submit your request online.
Step 5: Wait for a confirmation message from the Pharmacy Team that your medicines are ready for pickup.
Step 6: Visit the QPHN–Doña Marta Pharmacy on your scheduled pickup date and time. Present:
- Valid ID
- Original prescription (if needed, for validation)
- Reference or confirmation message

*Quantity of medicines dispensed is subject to stock availability.
*Patients will be notified once their request has been reviewed and approved for pickup.
*Yakap-eligible and Yakap-enroled patients must personally pickup their medicines, as photo consent is required.
