Between 40-70% of all antibiotic prescriptions worldwide are written without a confirmed bacterial diagnosis. The consequences: 39 million preventable deaths projected by 2050, $159 billion in healthcare costs, and a global economy $1.7 trillion smaller. South Asia sits at the epicentre, where 79% of patients in Nepal buy antibiotics over the counter without prescriptions, and nearly half believe fever requires antibiotics. Clinicians know the guidelines. They prescribe against them anyway because of patient pressure, diagnostic uncertainty, and systems that make evidence-based restraint structurally impossible. The post-antibiotic era isn't a distant threat. Drug-resistant typhoid, untreatable gonorrhea, and carbapenem-resistant ICU infections are current clinical realities. We have the evidence. We know the solutions. The question is whether we'll act in time.
